Automatic Monthly Payment Service
You can use our Automatic Monthly Payment Service form to sign up for our free automatic payment service. Just follow the steps below.
1. Click the Automatic Monthly Payment Service link above to open the form in a new window (you can still view these instructions).
2. Do one of the following:
Fill out all of the blanks in the bottom part of the file, then print the form by clicking File>Print.
or
Print a blank form by clicking File> Print and fill in the blanks by hand.Note: In either case, you will need to print the form and mail it to BCI to ensure the security and privacy of your personal information. Don't forget to sign the form where indicated.
3. Then, either:
Attach a voided check and mail the completed and signed form to us with your next payment.
or
